Example of an extended triangle test
This form contains:
- A triangle test.
- A line scale for the scoring of the intensity of the difference.
- Yes-No type questions for the description of the difference.
- Multiple-choice questions for the description of the difference.

Example of a monadic profile with line scales
This form contains:
- A monadic profile for 3 products split over 3 pages (only the first page is shown). The scales contain numbers that are optional.
- A judge identification where the number and name of the judge are printed (the tasters participating to the session were known in advance).

Example of a comparative profile with discrete scales
This form contains:
- A taster identification by number (the taster must write his number).
- The print date and the name of the questionnaire.
- A comparative profile for 6 products, scored on discrete scales.
- A multiple-choice question for the same samples and following the same evaluation order.
